Scents-ible Solutions Aromatherapy Program is a contributor (page 9 of the report) to the CMS Sharing Innovations In Quality web site. The Report, Development of the Artifacts of Culture Change Tool, describes the results of a CMS project released 4/21/2006 to develop a self-evaluation questionnaire. The second is The Artifacts of Culture Change Tool which is the self-evaluation questionnaire containing 79 items that represent actual changes that culture changing homes have made.
